Submit an online report



The Ageing and Disability Abuse Helpline is currently experiencing high demand. All reports are assessed and prioritised based on risk and urgency. After receiving your report via webform, phone or email, we will assess the information and determine what action to take. If the report is not assessed as urgent, you will hear back from us within four weeks. 

Please note: Our office will be closed from Monday 23 December 2024 to Friday 3 January 2025 for the Christmas and New Year period. We return on Monday 6 January 2025. During this time, the Ageing and Disability Abuse Helpline will be available on:

  • Monday 23 December
  • Tuesday 24 December 
  • Monday 30 December 
  • Tuesday 31 December 
  • Thursday 2 January 
  • Friday 3 January 

You can make a report to the Helpline on 1800 628 221 (9am to  4pm), or email helpline@adc.nsw.gov.au or by using the online report on this page. 

In an emergency, call 000. 

Before you make your report

  • If this is an emergency please do not use this form – please contact emergency services for urgent medical or police assistance
  • Be prepared with relevant information including – the name, date of birth, phone number and address of the person you are concerned about. If the person is currently in hospital please include this information, including the name of the hospital in the report.
  • Please include all relevant information about current abuse, neglect or exploitation concerns so we can assess the matter.
  • Please include your phone number or email address so we can send you a case reference number and clarify any details. Your identity is protected under law. 
  • For concerns about an NDIS, aged care or healthcare provider – please contact the relevant oversight body listed below:

  3. Provide as much detail as you can regarding the matter. While you can provide an anonymous report via this form, we would encourage you to call the Ageing and Disability Abuse Helpline on 1800 628 221 (Monday - Friday, 9am - 4pm) if you prefer to withhold your name.
