If you have a complaint about a NSW Government agency – please contact the agency directly to lodge your complaint or seek advice from the NSW Ombudsman.
The online report form will open in a new, secure window.
We do not routinely contact people who report to us to advise of the outcome of the report. If you would like advice about our assessment or the progress of the report, please contact us on 1800 628 221 and have your case reference number ready. The information we can provide may be limited due to privacy reasons.
Please notify us if the risk of harm to the older person or adult with disability increases due to abuse, neglect, or exploitation, or if new information arises that heightens these risks. Please send this information by emailing Helpline@adc.nsw.gov.au with your case reference number in the subject line or call our Helpline on 1800 628 221. There is no need to submit a new online report.
For immediate assistance, or to report an alleged criminal matter or seek urgent medical care, please call 000.
